The complaint also alleged improper staff-to-child ratios as well as inappropriate staff interactions with children during water play. An investigation was conducted on August 18, 2025. An investigation was conducted August 26, 2022. The complaint was substantiated because LS Brady observed a small amount of mouse droppings in between the cabinets on the floor of the kitchen. There was no evidence of mice eating food being served to children. A concern noted was the kitchen s base cabinets were not clean.

An investigation was conducted on May 16, 2022. There is no evidence to substantiate that staff-to-child ratio were incorrect because children s and staff attendance records were reviewed and found to be compliant. A concern was noted because the attendance sheets did not accurately reflect the children s location after naptime. Tracking sheets must reflect where children are at all times. Administrator Savanna Hitch, confirmed that she did not properly fill out the attendance sheet for this day.
