At the time of this notification, LS #1 received relevant documentation regarding this injury. On May 15, 2026, Office of Child Care Licensing (OCCL) received a complaint regarding allegations of inadequate playground supervision resulting in an injury to Child #1 during outdoor play activities. An investigation was conducted on May 26, 2026. On December 11, 2025, the Office of Child Care Licensing (OCCL) received a complaint concerning the administrative team failing to follow proper exclusionary procedures related to the identification of head lice within the program. An investigation was conducted on December 17, 2025. Based on the investigation, there is no evidence to substantiate this complaint for the program failing to follow exclusionary procedures related to head lice. Documentation obtained from the Brightwheel application indicated all three identified children were excluded from care for a minimum of 24 hours after beginning treatment for head lice.

On October 29, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding a failure to notify parents of an injury as well as a lack of supervision. An investigation was conducted on November 5, 2025. Based on the investigation, there is no evidence to substantiate the complaint of failure to notify the parents. The investigation determined the program was unaware of any injury when Child #1 was picked up from the program on October 23, 2025.
