On June 12, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding a lack of positive behavior supports. An investigation was conducted on June 15, 2026. There is no evidence to substantiate this complaint because staff members who were interviewed stated they have never observed a staff member hitting a child. Additionally, two children who are enrolled in the preschool classroom stated the teacher would give them a time-out and call their parents if they did something bad. The children also stated that they have never been hit or observed a teacher hitting other children.

OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. The Office of Child Care Licensing received a complaint on October 16, 2025, regarding a lack of positive behavior supports. An investigation began on November 12, 2025, and concluded on December 02, 2025. There is no evidence to substantiate this complaint because Staff #2 and Staff #3 stated they never saw Staff #1 pull the arm of any child. Staff #1 was not available to be interviewed; however, video footage and information were reviewed by another agency, and it was determined there were no charges regarding the incident.

This complaint is substantiated because according to attendance records, on July 1, 2022, July 5, 2022, July 6, 2022, July 7, 2022, July 8, 2022, and July 11, 2022, Staff #1, an aide, was alone with children in the one-year-old and two-year-old classroom. Also, during the visit, two staff members who are aides were alone with five children. Additional concerns noted were attendance records for staff and children were not accurately documented on several occasions.

LS Donovan and Smith investigated the complaint on August 27, 2021. There is no evidence to substantiate this complaint because the specialists did not observe any signs preventing the center from using the off-site playground. Additionally, the specialists observed all classrooms were following proper staff-to-child ratio. Ms. Smith also stated the center complies with the staff-to-child ratio in and out of the center.
