  On December 11 and 12, 2025, the Office of Child Care Licensing (OCCL) received two complaints regarding concerns of improper staff-to-child ratio, lack of supervision, and use of cell phones while caring for children. An unannounced visit was conducted on December 23, 2025. On January 6, 2026, a follow-up unannounced visit was conducted. This complaint is substantiated for improper staff-to-child ratio and exceeding maximum group size because upon arrival on December 11, 2025, there were 22 children ranging in age from infants to preschool-age gathered in one classroom.   The Office of Child Care Licensing (OCCL) received a complaint on May 14, 2025, alleging inappropriate discipline and lack of supervision. An investigation was conducted on May 30, 2025, which included an interview with Administrator #1, interviews with two staff members, and observations of the program and classrooms.   On December 7, 2023, a complaint was received by the Office of Child Care Licensing regarding concerns of lack of supervision, incorrect staff-to-child ratio, and improper use of screen time. An unannounced visit was conducted on December 8, 2023. This complaint is substantiated because Ms. Turner acknowledged inadequate staffing. Ms. Turner described her understanding of DELACARE regulations regarding screen time and required staff-to-child ratios when children of mixed ages are combined, but this information was incorrect.   On June 30, 2023, a complaint was called into the Child Abuse and Neglect Report Line and not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing received a second complaint and screened both in for investigation on July 5, 2023, regarding failure to report an injury and lack of supervision. There is no evidence to substantiate this complaint because, during the investigation, and subsequent written statements, Administrator Brooke Turner and Staff #1 both stated they were not aware of Child #1 receiving an injury until Parent #1 contacted the center.   The Office of Child Care Licensing received a complaint on June 20, 2023, regarding failure to report an injury to a parent and failure to follow proper handwashing and cleaning procedures. There is no evidence to substantiate this complaint for not reporting an injury because Ms. Turner stated she and the staff were not aware of an injury received at the center on June 16, 2023. There is no evidence to substantiate this complaint for improper cleaning and handwashing, as staff stated they followed the required cleaning procedures, and a visual inspection of the center did not identify any concerns about cleanliness.

An investigation was conducted on May 3, 2023. This complaint is substantiated because Staff #1 s written statement indicated she posted a picture of an enrolled child on social media which is in direct violation of the center s policy.

  The Office of Child Care Licensing (OCCL) received a complaint on August 18, 2022, regarding failure to provide notification/documentation for an accident or injury. In addition, several phone conversations and documents were received by OCCL during the following days regarding this concern. An investigation began on August 26, 2022, and concluded on October 24, 2022. There is no evidence to substantiate this complaint for failure to provide notification/documentation for an accident or injury because no staff was aware of any injury sustained by Child #1 on August 18, 2022.

  On August 1, 2022, OCCL received a complaint alleging discrimination and a placement denied based on a child s special educational needs/disability. The reporter stated Child #1 was initially accepted, then denied care based on her level one autism diagnosis. Based on the statements made by the administrator during the investigation visit, this complaint is substantiated. Ms. Turner stated Child #1 was initially accepted, then after further discussion, was denied enrollment because of the current staff s lack of training in the needs of children with autism.   On July 6, 2022, OCCL received a complaint alleging lack of supervision. The reporter stated Adult #1 stated Child #1 claimed she was touched sexually by another child during naptime, and Adult #1 stated to the center staff that Child #1 was touched during circle time. An investigation was conducted July 11, 2022. Based on the investigation, there is no evidence to substantiate this complaint. Video was not available, the times of alleged lack of supervision conflict, and the staff interviewed stated children are always supervised. All staff stated they were unaware of any inappropriate interaction between the two children.
