Based on interviews and documentation reviewed this complaint is substantiated for failing to follow infant feeding requirements because a staff member stated they sometimes feed an infant while the infant is on the floor. A picture in the Bright Wheel app documented an infant drinking a bottle while on the floor on May 6, 2026. On April 20, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding failing to follow child suspension/expulsion policy, failing to follow policy for handling complaints, and not following staff-to-child ratio. An investigation began on April 24, 2026, and concluded on April 29, 2026. Based on video footage viewed and documentation collected, there is no evidence to substantiate this complaint for failing to follow child suspension/expulsion policy. The Office of Child Care Licensing (OCCL) received a complaint on July 17, 2025, regarding concerns of lack of supervision. An investigation was conducted on July 21, 2025. There is no evidence to substantiate this complaint for concerns of lack of supervision based on the staff interview, staff statements reviewed, and video footage reviewed. Staff interviewed and staff statements consistently noted Child #1 lost his balance at the top of the slide and fell down the slide with staff present and attending to children. LS #1 observed video footage of the playground with two staff members engaging in play with nine children.

The Office of Child Care Licensing (OCCL) received a complaint on October 25, 2024, regarding concerns of failure to follow illness exclusions. A telephone investigation was conducted on October 28 and 29, 2024. This complaint is substantiated because a screenshot of Staff #1 s conversation with Administrator #1 shows Staff #1 state they had a fever along with Child #1, and Administrator #1 s response was that they would tell Staff #1 to stay home, but they were already short a staff member for the day. Staff #1 then stated they and Child #1 would take medication and come in.

On May 14, 2024, the Office of Child Care Licensing received a complaint regarding not following the center s breastfeeding policy by not allowing a parent to breastfeed at the center. There is no evidence to substantiate this complaint because the center administrator and Staff #1 both denied not allowing a parent to breastfeed at the center. The center administrator also provided LS Flowers with a copy of the center s breastfeeding policy and showed LS Flowers the private breastfeeding lounge.

The Office of Child Care Licensing (OCCL) received a complaint on June 27, 2023, regarding concerns of lack of supervision and not meeting the needs of a child. An investigation was conducted on June 27, 2023, and July 11, 2023. There is no evidence to substantiate this complaint because Child #1 was supervised, the staff member saw the fall, helped Child #1 up, and held his/her hand while he/she returned to play. Observation of the video footage from June 20, 2023, showed Child #1 continuing to play without showing signs of being injured.
