An investigation was conducted on June 17, 2026. Based on the investigation, the complaint is substantiated for the use of portable training potties within a classroom that did not have a bathroom. Licensing Specialists (LS) #1 and #2 observed two portable potty seats positioned within the Preschool 2 classroom. On April 10, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding failure to communicate important information relating to a child with the parent/guardian. An investigation was conducted on April 16, 2026. There is no evidence to substantiate this complaint based on interviews conducted and documents reviewed. While no communication was available requesting permission to use a weighted lap pad for Child #1, there is no current DELACARE Regulation requiring this. When the facility became aware Parent #1 did not approve of the weighted lap pad, the facility apologized to Parent #1 and discontinued its use.

On January 19, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding the program s failure to adequately report an injury requiring outside medical attention to OCCL within 24 hours following notification by the child s family. An investigation was conducted on January 28, 2026. Based on the investigation, this complaint is substantiated. While the administrative team provided documentation to the family and attempted to submit injury reports to OCCL via fax within the required timeframe, the program did not contact OCCL by phone to speak with an OCCL staff member to verbally report the injury as required in DELACARE Regulations. An investigation was conducted on July 31, 2025. Based on the findings, this complaint is substantiated for the use of screens with children under the age of two years old. Administrator #1 confirmed an infant teacher propped a tablet on a door handle while cleaning a classroom at the end of the day. There is no evidence to substantiate the complaint for the infant aged children being strapped in a stroller in the hallway unsupervised. On October 14, 2022, OCCL received a complaint regarding a lack of supervision. An investigation was conducted on October 17, 2022. Based on the results of this investigation there is no evidence to substantiate this complaint because LS Flowers and Budin reviewed video footage regarding this incident which showed Staff #1 was appropriately supervising the classroom, but was attending to another child when the incident occurred. Also, the center administrator and Staff #3 stated Staff #2 observed one of the incidents as it occurred but was too far away to stop it.

On August 4, 2021, OCCL received a complaint regarding lack of sanitation and failure to notify parents of an outbreak of communicable disease. An investigation was conducted on August 11, 2021. There is no evidence to substantiate this complaint for lack of sanitation because LS Flowers and Parchem observed the classrooms, toys, and equipment to be clean. Also, a procedure for cleaning toys and equipment was observed. There is no evidence to substantiate for failure to notify parents of an outbreak of communicable disease because a notice to parents was posted in the center.
