On June 28 and 29, 2022, OCCL received two complaints regarding FCCH provider Lakisha Webb regarding lack of supervision. An investigation was initiated on June 30, 2022, by phone and an investigation visit was conducted on July 7, 2022. Based on the results of the investigation, this complaint is substantiated because Ms. Webb left a three-year-old child unattended in the park for at least 45 minutes. Ms. Webb left the park and did not notice the child was not with her. Other non-compliance items were also noted during the visit because attendance records were not complete, Ms. Webb was over capacity by having four additional children, and Ms. Webb provided conflicting information to the po…
