On May 29, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding not informing parents of accidents or critical incidents involving the child and other important information relating to the child. An investigation was conducted on June 4, 2025. Based on documentation, video footage, and staff interviews, there is no evidence to substantiate this complaint. All staff interviewed stated they did not notice Child #1 s chipped tooth after the accident and Child #1 did not let staff look in their mouth to inspect it for injuries to the mouth or chips to the teeth, though it was noted there was no bruising or blood.

On May 11, 2023, a complaint was called into the Child Abuse and Neglect Report Line and not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing (OCCL) received the complaint on May 11, 2023, and accepted it for investigation on May 12, 2023, regarding improper discipline. An investigation was conducted on May 16, 2023. This complaint is substantiated for improper discipline because the staff member told the administrator and another staff member that she pushed the child because he scratched her. The center immediately terminated the staff member.
