On May 2, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding not following accident and injury procedures. An investigation was conducted on May 2, 2024. This complaint is substantiated because although staff were unaware of scratches occurring on Child #1 s torso, the accident/injury report for a separate injury occurring on the same day for this child was not signed by the parent/guardian. Other concerns noted was a lack of paper towels or hand dryer in the toilet room for children to dry their hands and the center s front door was unlocked, allowing anyone to enter the building without proper security procedures. An investigation was conducted on March 15, 2023.
