OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. On December 6, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding a lack of supervision. This concern had been self-reported to OCCL by the facility on December 2, 2024. An investigation was conducted on January 7, 2025. Based on the results of the investigation, staff interviews, and a review of documents and video footage, there is no evidence to substantiate this complaint for a lack of supervision. The staff person was present in the room but could not reach the child in time to prevent the fall.

On April 22, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision, improper staff-to-child ratios, improper mixing of age groups, staff files not being on-site, an enrolled child not having a file on-site, and not having an administrator functioning in that role. The complaint is substantiated for lack of supervision based on interviews and video footage. Child #1 was observed on video to have been on the playground unsupervised for 25 minutes without staff knowledge. On August 24, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding the center not maintaining and cleaning the classrooms, mixing age groups, not notifying the families of a communicable disease, no menu being posted in the center, and the food service not meeting CACFP requirements. An unannounced visit was conducted on August 31, 2023, to investigate this complaint. There is no evidence to substantiate this complaint because it was observed that the classrooms were clean, a cleaning schedule was present and being followed, the children were in the appropriate classrooms separated by age, and the current menu was posted with the corresponding food items present in …

On May 26, 2023, a complaint was made to the Child Abuse and Neglect Report Line and accepted for investigation by the Institutional Abuse Unit (IAU). IAU abridged the case on May 30, 2023, at which time the Office of Child Care Licensing (OCCL) received the complaint regarding an allegation of improper discipline. An unannounced visit was conducted on June 2, 2023, to investigate this complaint. There is no evidence to substantiate this complaint for improper discipline because all the staff members interviewed stated Child #1 was never alone with Staff #2 and the video footage which was reviewed confirmed this to be true. Staff #2 denied ever harming any of the children in care.
