An investigation was conducted on January 6, 2025. There is no evidence to substantiate this complaint. After interviewing staff members and administration the needs of the children are being met through daily routines. The lack of accessibility of a spoon on one particular day was the result of unclear communication between the child and staff.

On May 15, 2025, an incident of staff failing to use appropriate behavior supports was self-reported to OCCL by the facility. On June 3, 2025, the Office of Child Care Licensing (OCCL) received two complaints regarding the same incident. An investigation was begun on May 15, 2025, and concluded on August 13, 2025, after clearance from other agencies was received. An investigation was conducted on May 15, 2025. There is no evidence to substantiate this complaint for incorrect staff-to-child ratio. After reviewing attendance records and interviewing staff, all classrooms were found to be in correct staff-to-child ratio. There is no evidence to substantiate this complaint for unqualified staff members left alone as attendance records and staff interviews show the two unqualified staff members are not alone with children.
