On July 21, and July 29, 2025, the Office of Child Care Licensing (OCCL) received self-reports from the center regarding two separate incidents of a lack of supervision. On July 29, 2025, OCCL received a complaint about a lack of supervision regarding the July 21, 2025, incident. An investigation began on August 11, 2025, and concluded on September 2, 2025. Based on staff statements, administrative interviews, and observations at the visit, this complaint is substantiated for a lack of supervision. On July 18, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding an allegation of improper discipline. An investigation was conducted on July 23, 2025. Based on staff interviews, there is no evidence to substantiate this complaint for improper discipline. All staff members interviewed stated they have not witnessed or heard of any staff members using improper discipline with any children. Additionally, there was documentation of staff using positive reinforcement with the children.

On October 1, 2021, the Office of Child Care Licensing received a complaint regarding the center not following current COVID requirements. An investigation was conducted on October 6, 2021. There is no evidence to substantiate this complaint because all staff who were interviewed stated DPH was notified as required and staff and children who were required to quarantine were excluded from care. Also, LS Flowers and Parchem observed children and staff wearing masks properly.
