OCCL received a complaint on September 29, 2022, regarding unqualified staff being left alone, and insufficient materials in the classroom. An investigation was conducted on October 5, 2022. There is no evidence to substantiate the complaint because according to attendance records, there are were no unqualified staff alone with the children and LS Smith observed enough materials in each classroom. A concern noted was a lack of supervision because Staff #2 allowed a child to leave the classroom to use the bathroom twice.
