This complaint is substantiated for failing to use appropriate behavior supports based on the interview conducted and statements reviewed. Administrator #1 stated after reviewing Staff #1's statement, Staff #1 was not using appropriate behavior support while in the bathroom with Child #1. Statements reviewed from two staff members, and one parent consistently noted Staff #1 did not use appropriate behavior support when Child #1 had a bathroom accident.
