On March 11, 2022, a complaint was received by IA and not accepted for investigation. On March 14, 2022, it was sent to OCCL and accepted for investigation regarding parents concern of the accuracy of how an injury occurred to their child. There is no evidence to substantiate this complaint because all written and verbal accounts regarding how Child #1 was injured are consistent in explaining that the child was hit in the mouth with his heavy cup when attempting to drink from it. Staff #1 was present and witnessed the injury. No evidence has been presented to counter the staff member s statement. The children remain enrolled.

On November 9, 2021, OCCL received a complaint regarding Child #1 being given a food item to which he had a sensitivity. Based on the information received during the investigation this complaint is substantiated for failure to follow the center s and OCCL s food service policy. The parent brought in the food to the center, yet instructed the teacher not give it to Child #1. It was placed on Child #1 s plate in error. It is unknown if it was consumed by Child #1.

On November 10, 2021, OCCL received a complaint regarding a claim that a staff member stated to an individual touring the center that blankets were used to cover sleeping infants in swings. An investigation was conducted on November 17, 2021. During the investigation, an observation of the infant rooms and interviews with staff members occurred. Based on these observations and interviews, there is no evidence to substantiate the complaint.
