On October 23, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding a concern of not using positive behavior supports resulting in an injury to a child. An investigation was conducted on October 30, 2025. Based on the investigation, there is no evidence to substantiate the complaint based on interviews with staff members and review of video footage. Staff #2 denied using physical touch to assist Child #1 with changing clothes in the bathroom on October 15, 2025. OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. Between February 19, 2025, and February 20, 2025, three complaints were reported to the Office of Child Care Licensing (OCCL) regarding concerns of improper discipline. On February 18, 2025, Administrator #1 self-reported the incident to Licensing Specialist #1. An investigation was conducted on February 26, 2025. Based on the results of the investigation, this complaint is substantiated for improper discipline, as video footage reviewed by LS #1 and #2 showed Staff #2 grabbing Child #1 by one hand and arm and lifting them into the air on two separate occasions.
