Based on interviews and a review of documentation, this complaint is substantiated for lack of supervision and failure to follow transportation policies. A transportation log was not completed on March 23, 2026, and staff members did not conduct a thorough attendance check of children on the van.

On October 22, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of improper discipline. An investigation was conducted on October 23, 2024. This complaint is substantiated for improper discipline based on interviews and review of video footage from Staff #1 s classroom. Staff #1 was observed pulling a child across the carpet by one arm and was also observed smacking Child #1 on the side/bottom after Child #1 fell in the classroom. Staff #1 s employment was terminated on October 21, 2024, after these allegations were brought to the facility s attention and video footage was viewed.
