On September 9, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding accident/injury procedures and lack of supervision. An investigation began on September 10, 2025, and concluded on September 11, 2025. There is no evidence to substantiate this complaint because all staff interviewed stated the mark on Child #1 s face appeared to be a mosquito bite that he/she was scratching, and no known injury occurred. Additionally, according to various video recordings from the one-year-old classroom on September 9, 2025, Staff #1 was observed moving around the classroom, engaging with, and supervising children. No bite or other injury was observed.
