On November 22, 2022, the Institutional Abuse Unit (IAU) received a complaint regarding a child injury but did not accept it for investigation. The Office of Child Care Licensing (OCCL) received the complaint on November 28, 2022, and accepted it for investigation for allegations of lack of supervision. An investigation began on December 1, 2022, and ended on January 24, 2023. The complaint is substantiated for lack of supervision and incorrect staff-to-child ratio, because staff were observed on video from November 21, 2022, leaving children unsupervised and classes out of ratio. On August 31, 2022, OCCL received a complaint regarding classrooms having improper staff-to-child ratios, classrooms exceeding maximum group size, and unqualified staff alone with children. The complaint also alleged a child was left alone in a classroom for 20 minutes one day in May, 2022. An investigation was conducted, beginning on September 6, 2022, and ending on September 13, 2022. There is no evidence to substantiate the complaint because all classrooms were in correct staff-to-child ratios at the time of the visit and no classroom exceeded maximum group size.
