    On January 16, 2026, the Office of Child Care Licensing (OCCL) received a complaint alleging a lack of supervision in the classroom, resulting in preventable injuries sustained while a child was in attendance at the program. An investigation was conducted via phone call and email communication on January 29, 2026. Based on the investigation, there is no evidence to substantiate the complaint. Administrator #1 provided injury reports and video footage to Licensing Specialist (LS) #1, which depicted classroom staff engaging with children and intervening appropriately when a conflict between Child #1 and another child in the classroom occurred.

    On April 3, 2024, a complaint was called into the Office of Child Care Licensing (OCCL) regarding improper discipline. OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. An investigation was conducted by OCCL on June 17, 2024. Based on the results of this investigation this complaint is substantiated for improper discipline because video footage observed showed Staff #1 pushing Child #1 to the ground, pulling Child #1 across the floor, and hitting Child #1 in the leg with a bookshelf.     On March 10, 2024, The Office of Child Care Licensing (OCCL) received a complaint regarding concerns of inappropriate care of children. This incident was also self-reported to OCCL on March 8, 2024. This complaint is substantiated because a staff member was holding two infants at one time and attempted to open the classroom door. Child #1 slipped out of Staff #1 s arms and fell, hitting their head on the floor. This resulted in a bruise on the head.     On February 16, 2022, the Office of Child Care Licensing received a complaint regarding a child in care alleging a staff member put her hands around Child #1 s neck. At the conclusion of an investigation conducted on March 14, 2022, there is no evidence to substantiate this complaint. There were no marks present on Child #1, and the staff members in the classroom stated they have not witnessed any other staff or children place their hands around the neck of any child. The staff at the center did note that Child #1 does have behavioral concerns and staff have had to hug her to calm her down.
