An investigation was begun on June 23, 2025, and concluded on September 9, 2025, after clearance from other agencies was received. The Office of Child Care Licensing received a complaint on November 20, 2024, regarding unsanitary conditions in the center. An investigation was conducted on November 25, 2024. On April 25, 2024, Staff #1 brought a vape pen into the classroom where it was in Child #1 s possession for two minutes without the staff member s awareness.

On December 4, 2023, a complaint was called into the Child Abuse and Neglect Report Line and was accepted for investigation by the Institutional Abuse Unit (IAU). It was sent to the Office of Child Care Licensing (OCCL) on December 5, 2023, and accepted for investigation for lack of supervision. IAU closed their case on January 30, 2024. Law enforcement was involved and OCCL received permission from the New Castle County Police Department (NCCPD) to begin an investigation on January 24, 2024.
